The affidavit requires the filer to select one response about nonparties with physical custody or claimed custody or visitation rights.

Update the Clerk of Courts if any of the listed contact information changes.

Parties must initiate a telephone conference with the Court before filing any motion to compel.

Only one captioned document may be included in each E-Filing, but attachments to that document may be included in the same filing.
